Processing Times:

  • Routine: 8-11 Weeks
  • Expedited: 5-7 Weeks

Both parents of the passport applicant must be present at the time of appointment unless one of the following can be presented by the applying parent/guardian:

  • Form DS-3053 (notarized within the last 90 days) and a front and back copy of the non-appearing parent/guardian driver’s license or identification card; or
  • Court order granting sole legal custody to the parent (unless child’s travel is restricted by that order); or
  • Judicial declaration of incompetence of the non-appearing parent; or
  • Death certificate of the non-appearing parent

The passport applicant must:

  1. Be present at the time of the appointment; and
  2. Provide a completed (but not signed) Form DS-11, and
  3. Provide one passport photo or have one taken at the City of Roseville for an additional fee of $12. (As of November 1, 2016, eye glasses are not permitted when taking a photo for a U.S. passport.)
  4. Provide proof of U.S. Citizenship (original/certified copy) via:
    • U.S. birth certificate issued by the City, County, or State of birth which:
      • Lists bearer's full name, date of birth, and place of birth
      • Lists parent(s) full names
      • Has date filed with registrar's office (must be within one year of birth)
      • Has registrar's signature
      • Has embossed, impressed, or multicolored seal of registrar
    • Adoption Decree; or
    • Consular Report of Birth Abroad; or
    • Original Certificate of Naturalization
    • Provide proof of parental relationship (original/certified birth certificate)
  5. Parent(s) of the passport applicant must provide valid photo identification (and a clear photocopy showing the front and back of the photo identification) in the form of one of the following:
    • Valid, undamaged U.S. passport; or
    • Certificate of Naturalization; or
    • Valid Driver's License (plus second ID if issued in a state other than California); or
    • Current Military ID
  6. Make payment for fees as follows:
    • A non-refundable application fee (payable to the U.S. Department of State) by personal check or money order only). See table below for specific fee.
