  • Spec sheet listing UEF rating
  • Paid-in full invoice including contractor/retailer information and product make and model numbers. Invoice must show total cost of project without rebates, any rebate amount advanced, permit fees and net balance due from customer.
  • Completed and signed application
  • City of Roseville issued building permit
